Gajda Health Plus Missed Appointment Policy
If you are unable to keep a scheduled appointment, please notify our office at least 24 hours prior to your scheduled appointment. Voice mail is available after office hours to leave a message. Failure to do so could result in a $25.00 cancellation charge being billed to you. This also applies to telephone consultations. Please notify us of a change in your schedule as soon as possible as this allows us to make your appointment time available to other patients. Thank you for your cooperation in this matter.
